Insurance and Billing Information

We accept most insurance plans and are in-network providers with:
  • Medicare (Traditional Medicare only)
  • Blue Cross Blue Shield of Texas
  • HealthSmart PPO
  • CIGNA
  • Aetna (TRS Plans Only)
  • United Healthcare
  • Beechstreet
  • Multiplan
  • PHCS
  • Tricare Standard 
  • Tricare Prime (with referral from PCM)
  • Tricare Active Duty (with referral from PCM)
  • Unicare
If your insurance is not listed, please call our office so we can check your benefits. 

We do not accept:
Worker's Compensation
Automobile Accident
Personal injury cases
Any open litigation or pending litigation
Medicaid
Limited Benefit Policies 
Cash pay patients 
Tribal Healthcare Reimbursement 

We do require estimated deductible (up to insurance allowable amount for service), co-payment, co-insurance to be paid at the time of service.  Patient financing is available through Care Credit. 

Our physicians are considered Specialists. If your insurance plan requires a referral to receive the highest level of benefits, it is the patient's responsibility to request and maintain a current referral for services rendered at The Pain Rehabilitation Group.  As a courtesy, our staff will try to notify you if you need a referral prior to your next appointment. If a current referral is not in place at the time of appointment check in, the patient will be required to pay the full estimated amount due at the lower level of benefits.
